‘Careforce’ neighbourhood mapping – releasing time to care Torbay Council

The lack of domiciliary care capacity is a national challenge. This project aims to address this challenge in Torbay, and to develop a model and toolkit that can be shared more widely across social care and health systems.

The project, led by Torbay Council, will build on initial work underway with Whole Systems Partnership to develop a ‘Careforce’. The Careforce concept is based on the idea that it is possible to identify and share data relating to all those engaged in or available to facilitate care and support in a particular area, irrespective of the organisation that they work for. This supports the delivery of more effective and efficient working arrangements that are focused on the needs of local people.

The project team will use data held about care provision to develop a common understanding of the needs, expectations, outcomes and costs of supporting those in receipt of home care in a specific geographical location.

The project will build on initial work that was developed for the local snow plan, and served well for the COVID-19 crisis response. A better understanding of the ‘who, what and where’ of need and capacity will create increased resilience and support contingency planning in the short term.

The core mapping package is likely to be embedded in other tools, using repeatable, open source packages. The team will also produce templates for sharing workforce data and intelligence; data definitions and outcome measures that reflect the lived experience of people in receipt of social care; and a framework, language and initial data map for identifying and mapping diverse resources at a neighbourhood level.
